Part 1: The appointment prep sheet

Appointments are short, and it is easy to leave with half your questions unasked. This one-page sheet helps you get ready in ten minutes, before a doctor, specialist or clinic visit. Print it and take it with you.

Before the visit (the night before)

  • Why am I going? Write one sentence: ______________________
  • Write down my top three concerns, most important first. If time runs short, you will have covered what matters.
  • Note the details: when it started, how often, what makes it better or worse, what I have tried.
  • Gather my medicines. List names, doses and how often, including vitamins and supplements. Or bring the bottles.
  • Bring my insurance card, photo ID and any referral or forms.
  • Bring results or records from other doctors if I have them.
  • Ask someone to come if I would like a second pair of ears, or ask if I can use a phone for notes.
